- dimensional repositioning of the jaws feasible, more precise, and more predictable than had been possible with traditional methods. Individualization of osteotomy designs based upon anatomic, biomechanical, biologic, functional, and esthetic criteria has contributed significantly to the development of new surgical procedures with more predictable, stable, functional, and esthetic results.
Rigid internal fixation has been well accepted by patients and most professionals actively involved in orthognathic surgery. The use of bone plate and screw osteosyn
thesis has virtually eliminated the need for maxillomandibular fixation and dramatically altered postsurgical neuromuscular and occlusal rehabilitation. Additionally, the concepts of selectively altering the occlusal plane, managing short mandibular ramus deformities, and rotating the mandible counterclockwise have come of age through the modification of rigid internal and skeletal fixation techniques.
Properly planned and precisely positioned osteotomies in concert with orthopedic appliances facilitate an increase of the arch length in both the maxilla and the mandible and permit surgical orthodontic treatment of the majority of dentofacial deformity patients, without extraction of teeth. Such therapy greatly improves treatment efficiency and effectiveness.
Charles Lindbergh: "What kind of men would live where there is no daring? I don' t believe in taking foolish chances, but nothing can be accomplished without taking any chance at all.”
A specialty derives its standards from selected individuals who emerge to take their place in
history. I have attempted to select some of these individuals who have made and will continue to make a significant impact on the specialty of oral and maxillofacial surgery. Each contributor is active and dynamic in a particular field of interest.
Volume l, 2, and 3 of Modern Practice in Orthognathic and Reconstructive Surgery describe the work of visionaries, creative thinkers, and pioneers who dared to take "the first step."
Through their efforts, our specialty continues to move forward to new heights.
As a family of oral and maxillofacial surgeons, we possess the powerful force of our specialty spirit. We must spread to people throughout the world the good things we have learned through our parents, our teachers, and our colleagues. The world stands to benefit from all the values we have been so fortunate and privileged to learn as professionally trained people do.
Haughty, selfish specialty pride and "turfbuilding" limit progress and 'the good that can be done. The surgical procedures and methods of treatment (which must be correctly taught) described in these books should be available to all patients and specialists throughout the world. May these books contribute to this end?
Science is a universal language successfully spoken at alt international meetings. We live in an age of immediate communications. That the knowledge found in these books will touch people the world over is an exciting thought that prompted me to pursue the writing and editing of these three volumes containing information from contributors all over the world.
In the course of reading these books, it is hoped t
