• Patient has history of nausea and vomiting.
GENITOURINARY SYSTEM
• Usually urinates 3-4 times daily.
• No hazards of urinating during night.
• No history of difficulty in urinating, anuria, and polyuria.
REPRODUCTIVE SYSTEM
• No history of any sexually transmitted disease.
• No menarche yet.
• No problem regarding her reproductive system.
HEMATOLOGIC
• Patient does not easily bruise, and bleed.
• No history of blood transfusion.
• Patient is anemic as evidenced by the results reflected at the laboratory exams.
ENDOCRINE
• No history of thyroid problem, heat intolerance, cold intolerance, excessive sweating, excessive thirst, excessive hunger, and polyuria.
PSYCHIATRIC
• No history of depression and suicidal attempts.
• No history of antipsychotic drugs consumption.
