REVIEW OF SYSTEMS 
GENERAL APPEARANCE 
Patient’s usual weight is 25 kgs and its recent weight is 24 kgs. She is unconscious, level of consciousness in a comatose state with a Glasgow coma scale of 6. 
INTEGUMENTARY SYSTEM 
• Patient has history of lesions and her mother used betadine solution to clean the wound. 
• Aside from skin lesions, patient has no other history of skin problems, so with her family. 
• No hair problems, but lice are present. 
HEAD, EYES, EARS, NOSE THROAT (HEENT) 
Head 
• Patient had a history of slight headache. 
• No history of head injury. 
Eyes and Vision 
• Patient had no history of eye problems, and she has good eyesight before the illness 
• Just during the course of her present illness she’s not reactive to light. 
Ears and Hearing 
• Patient has no history of hearing problems and any abnormalities or disorders involving her ears. 
• During her present illness, patient do not respond to verbal command 
Nose and Sinuses 
• Patient has no history of problems regarding her sense of smell 
• Has history of cold 
Mouth and Oropharynx 
• Patient has no history of any mouth diseases. 
• No history of bleeding gums. 
• No annual visit to any dentist. 
• Daily dental care was three times daily. 
• No dentures. 
RESPIRATORY SYSTEM 
• No history of asthma attack. 
• (+) crackles 
• No family history of weak lungs. 
• (-) cough 
CARDIOVASCULAR SYSTEM 
• Has family history of hypertension specifically her grandmother on father’s side. 
• No history of weak heart. 
• No underlying diseases that affect the heart such as obesity, diabetes, lung disease and endocrine disorder. 
• No systems indicative for heart disease such as palpitations, syncope, chest pain, and edema. 
NERVOUS SYSTEM 
• Has a convulsion last July 14, 2008 one day prior to admission. 
• Patient is unconscious during admission but prior to that she has good consciousness. 
• Patient has no sensitivity to light. 
• No history of blackouts. 
• No previous history of (+) Brudzinki’s and Kernig’s sign just by now during the course of her illness. 
MUSCULOSKELETAL SYSTEM 
• Patient has no previous injuries. 
• During hospitalization there’s (+) Brudzinki’s and Kernig’s sign. Onset of pain was sudden. 
• Patient was able to perform ADL’s before the course of her illness. 
• During hospitalization patient has limited movement and most of he time just lying on bed. 
GASTROINTESTINAL SYSTEM 
• No history of serious abdominal pain. 
• No history gallbladder and liver problem. 
• No history of hematemesis. 
• No history of fecal incontinence.
